All attendees must must provide proof of Flu vaccination in one of the following ways below:
1. Letter/Certificate from GP/pharmacist: a letter or a certificate from a doctor or pharmacist which shows your name and the date of your flu vaccination.
2. Invoice: an invoice from a doctor or pharmacist which shows your name and clearly shows that the payment was for flu vaccination.
3. Medicare Online: a screenshot, photograph or print-out of your Medicare online immunisation history showing you have received the flu vaccination and the date you received it.
4, Letter/Certificate from employer: a letter or certificate from your employer or a commercial vaccination provider engaged by your employer to provide flu vaccination, showing your name and the date of your flu vaccination.
